DeuceSSH: add API documentation

README.md with complete client and server usage guides, I/O callback
contract, algorithm registration, DH group provider, thread safety
model, error handling, and file layout.

Documentation comments on all public functions in deucessh.h,
ssh-trans.h, ssh-auth.h, and ssh-conn.h. Fixed duplicate comment
block in deucessh.h.
